New Patient Registration

registration1If you live in our practice area and wish to register as a patient with our practice then we need certain information from you and each member of your family so we can register you and obtain your previous medical records. 

You need to complete a GMS1 form (available below or from reception) which asks for information regarding your address and previous surgery details. You will also need to provide 2 forms of ID including photographic evidence of your identity e.g. a passport or driving licence and proof that you are living at the address you are registering at e.g. a utility bill.

To register you may call into the surgery, bringing with your NHS number which you can obtain from your previous GP. You do not need your NHS number to register with the practice it will just help to speed up your registration process. If you do not have an NHS number e.g, you have recently arrived in the country then we will ask you to give us some further details regarding your previous country of residence.

We offer all patients who register with the practice a New Patient Health Check, please ask at reception to book an appointment.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
